Information About Community Options

Find your ASAP!

If you or someone you know currently lives in a nursing facility and would like information about community living, follow these steps to start the conversation with a case manager:

  1. Find your ASAP
Press and hold the Ctrl key on your keyboard, then hit the letter f. A search bar should pop up. Type the name of the place where your nursing facility is located into the search bar to find your ASAP.
  2. Call the ASAP in your area and ask about the CTLP team
Someone from CTLP should schedule a visit to come and see you at your nursing facility to discuss community options or answer any questions you may have.
  3. If you have problems with the CTLP team, please email us at bringingpeoplehome@cpr-ma.org.

Information About Medicaid Waiver Appeals

If you or someone you know currently lives in a nursing facility and applied for a Medicaid Waiver and got denied, you can appeal that denial. Denials come by mail and should say the reason you got denied. The letter has important dates, including an appeal deadline. It is important to start the process or ask for help quickly after receiving the letter.
